Do I need a referral from my primary care physician in order to schedule a new patient appointment?

Although a referral from a PCP is not required, we do encourage records to be sent prior to the appointment time in order to get a better understanding of your current health condition. Please make sure insurance referrals are sent prior to the appointment date if your insurance requires you to have one.

What insurance plans are accepted?

All insurance plans are accepted, except for Medicaid.

What should I bring to my appointment?

We ask that all patients to bring a list of their current medications and, if diabetic, their blood sugar meter or log.

Yes, however the full cost is due at the time of the appointment. Please call our office for an estimate.
